MAIN FUNCTION:
Assumes the responsibility for providing and overseeing the patient plan of care to ensure care is specific to the patient’s needs and is delivered in an effective manner to achieve positive outcomes. The BH Case Manager will use techniques such as motivational interviewing, behavior change, and cognitive behavioral therapy techniques to help people achieve optimal health and well-being.
ESSENTIAL TASKS:
- Working hours: 8:00a.m. – 5:00p.m., reports patient status to the Clinical Care Manager/RN Supervisor after the last visit. Discusses alterations in the patient’s plan of care as needed.
- Evaluates patients, caregivers and home environment for correct OASIS data collection on the day that is assigned for the following assessments:
- Admission
- Recertification
- Resumption of Care
- SCIC Follow-up
- Transfer to Facility
- Discharge
- Calls the Clinical Care Manager from the patient’s home if needed to determine services and frequencies for the plan of care.
- Provides comprehensive nursing care through the utilization of home assessment, ICD-10 diagnoses application, plan of care development and plan of care oversight.
- Performs treatment(s) requiring specialized skills ordered by the physician.
- Observe signs and symptoms and reports them to the RN Supervisor and/or physician immediately.
- Notes changes in patient’s physical or emotional status.
- Teaches and supervises the patient and family members regarding the nursing care needs.
- Incorporates other team members into the care of patients, under the directions of the referring physician.
- Receives report from other discipline involved in the plan of care and gives direction for adjustments in the care.
- Use techniques such as motivational interviewing, behavior change, and cognitive behavioral therapy techniques to help people achieve optimal health and well-being.
- Receives orders care to be delivered or alterations in the plan of care.
- Determines the frequency of visit to ensure patient positive outcomes can be achieved as care is delivered.
- Addressed clarifications brought by the QAPI team timely to ensure care is accurate and specific to the patient’s needs.
- Ensures the patient and/or caregivers are provided information about patient’s rights and responsibilities while providing a clear understanding of the care to be delivered.
- Establishes priorities and goals for the patient and implements plans to achieve them.
- Coordinates care with all disciplines and ensures goals are achieved timely.
- Follows the Plan of Care as established in the HCFA Form 485.
- Updates the clinical record as necessary with new orders and communicates the changes to the patient and/or caregiver.
- Administers medication as defined in the medication policy, and treatments, as prescribed by a physician.
- Coordinates psychoactive treatments under the direction of the patient’s physician.
- Corroborates information with the physician to provide supporting documentation for the need for home care and face to face to be approved by the physician.
- Submits clinical documentation to the Quality Assurance department within 24 hours of visit. Resolves clarifications within 48 hours or sooner if dedicated by QAPI.
- Submits completed Admission forms within 24 hours of visit for review.
- Supervises home health aides within his/her service area or as directed by the RN Supervisor every fourteen days and document supervisory visits in the EMR.
- Consults with physician when necessary and documents changes in the EMR the with orders and submits within 24 hours of the visit.
- Participates in professional meetings when directed by the RN Supervisor with attention to punctuality and attendance.
- Attends scheduled inservices and case conferences.
- Develops and implements discharge plans for Agency patients, completing discharge change order after consulting with the Clinical Care Manager/ RN Supervisor and notifies the patient and physician in a timely manner.
- Keeps fully cognizant of pay source’s reimbursement criteria, current charting needs and dates of recertification of all patients under his/her care and supervision. All recent information must be turned in to the RN Supervisor/ Clinical Director 2 weeks before due.
- Prepares written care plans for Home Health Aides and updates at supervisory visit if needed.
- Rotates weekend call with the other staff nurses and complies with the policy and procedures for weekend call.
- Other duties as assigned by the Clinical Care Manager/RN Supervisor.
- Full-time RN Case Managers are required to make minimum visits as dictated by productivity standards communicated by management.
- Assists with the Orientation and on-going training of nurses and home health aides.
- Complies with departmental and company policies and procedures.
- Follow agency infection control processes and procedures to ensure a safe work environment.
- Other duties as required.
EDUCATION AND EXPERIENCE:
- Hold a master's degree in psychiatric or mental health nursing; or
- Hold a bachelor's degree in nursing with one year of full-time experience in an active treatment unit in a mental health facility or outpatient clinic; or
- Hold a diploma or associate degree with two years of full-time experience in an active treatment unit in a mental health facility or outpatient clinic; or
- for an RN for Medicare-certified agencies, as allowed by the fiscal intermediary for Texas contracting with the United States Department of Health and Human Services (USDHHS) CMS.
- Registered Nurse, currently licensed in the State of Texas.
- Graduate from an approved school of nursing.
- Maintains an active BLS certification and card at all times.
- One year of experience in an acute care setting that requires heart, lung, and complete physical assessment is preferred, but not required. Demonstrated ability or education in the same areas is preferred.
