Maximize Healthcare is seeking a full time, experienced nurse practitioner / NP to provide primary care services at a skilled nursing facility in the Bronx, New York. Our unique model provides Nurse Practitioners with the opportunity to build independent patient panels while working in collaboration with facility health care teams.

Maximize Healthcare is reinventing geriatric healthcare by filling care gaps that are commonly seen in this population. Our nurse practitioners provide high quality, evidence-based, compassionate care to geriatric patients and truly make a difference in the lives of patients and their families.

Responsibilities:

Provide primary care services to residents in assigned facility

Manage acute and chronic health conditions

Order and interpret diagnostic labs and imaging

Create treatment plans and prescribe medication as indicated

Collaborate with multidisciplinary healthcare team

Provide education and support to patients and families

Requirements:

Nurse Practitioner / NP (FNP or AGNP) with current NY licensure in good standing

2+ years experience as NP in long term care or similar setting

Nurse Practitioner / NP or RN experience in acute care setting such as ICU or ER preferred

Board Certification

Active DEA license

Ability to work autonomously and as part of a healthcare team
