The Role: *
The Patient Navigator will assist the clinical team by preparing patients for Telehealth visits prior to the initial consultation. This individual will leverage Assure Health’s technology while working collaboratively with a dynamic care team composed of Nurse Practitioners (NPs) and physicians. This role will be based in Assure Health’s Fort Lauderdale Lakes office or remote.

Responsibilities:

• Coordinate with patients and caregivers in advance of the Telehealth visit.
• Collect pre-visit information and pre-fill information in the chart.
• Coordinate the provider schedule and communicate patient availability to the provider
• Document necessary and applicable information about the patient by being clear and concise
• Manage provider schedules along with team members to ensure that providers stay on time and patients are seen promptly.
• Record patient information concisely, accurately, and completely
• Communicate program requirements and set expectations with patients and caregivers

Skills and Qualifications: *

• Patient-facing healthcare experience preferred
• Passion for improving patient outcomes long term
• Able to work full-time in-person at our Lauderdale Lakes, FL office or remote
• Strong communication skills and able to navigate team dynamics
• Proven experience being a quality, patient-focused caregiver
• Previous experience in care management and/or a background in pharmacy or medication management is preferred

Job Type: Full-time

Pay: $23.00 per hour

Schedule:

• 8 hour shift
• Day shift
• Weekend availability

Work Location: Remote
