LPN Responsibilities:

- Follow doctor’s orders for patient care as stated in the MAR/TAR

- Participates in the planning and coordination of total client care in conjunction with the RN, agency policies and the physician’s Plan of Care.

- Follows the Nursing care plan for each assigned client

- Accurately reports and records the client’s condition and care, including signs and symptoms which may be indicative of change

- Assists the physician and Registered Nurse in complex nursing situations and performing specialized procedures.

- Prepares Equipment and materials for treatments observing aseptic technique as required

- Observes the client; evaluates the care of the client; applies his/her knowledge of nursing skills to the clients to whom he/she is assigned

RN Responsibilities:

- Follow doctor’s orders for patient care as stated in the MAR/TAR

- Obtaining A medical history, particularly as it relates to present conditions, from the client and/ or family member

- Conducting a physical examination of the client, including vital signs, physical assessment, mental status, appetite, and type of diet, etc.

- Evaluating the client’s environment to determine what assistance will be available from family members in caring for the client

- Responsible for the care given by the Aide to clients in his/her care by:

- Supervising and evaluating the care given by the staff as needed.

- Coordinates total Client care

- Participates in the planning, operation, and evaluation of the nursing care.

- Prepares the care plan.

- Accurately reports and records the client’s condition and care, including signs and symptoms which may be indicative of change
